• We want to welcome you to your first visit with Bear Pediatric, PLLC.
• It is our goal that together we will form a team that will be with you as your child grows and develops. The following are a few tips to help you through those initial visits.
• Background information is very important, such as medical history, family history, surgical history, allergies and immunizations. Please help us by providing all this information beforehand. 
• Bring any list of medications with dosages so that we may record that as a part of your child’s record.
• If this is your first visit or you are coming for your well visit please bring all copies of your child’s vaccine record (even if you do not anticipate getting shots at that visit).
• Please see patient forms to download and complete prior to your visit.
